- Passport with at least 6 months (1 year for multiple entry visa) remaining validity on the date of entry into Indonesia.
- Applicants under 18 years of age should attach a notarized copy of birth certificate and notarized letter of consent signed by non-traveling parents/guardian.
Indonesia tourist visa:
- Itinerary (original/copy) or a letter from travel agents, airline, steamship company, confirming the purchase of tickets into Indonesia (Note the officer may ask the copy of the original airplane ticket).
- Letter of Employment, indicating the employment status of the applicant. For a retiree, please submit proof of the retirement fund.
- Copy of the most recent bank statement.
Indonesia business visa:
- A letter of intent (2 copies) from the employer or sponsor, describing the purpose of the visit and guaranteeing to cover all transportation and living expenses while in Indonesia.
- For multiple entries visa: a letter of invitation (2 copies) from business partner or sponsor in Indonesia describing the purpose of the visit.

Visas are valid for 90 days from the date of issue. Please apply only within 90 days of the date of travel.
Important: There are 3 regions in Indonesia that the applicant could not visit without special permission and approval. The three regions are: Aceh, Maluku, and Irian Jaya. Travelers who wish to enter the restricted regions must obtain special authorization from Indonesian authorities. Please advise the Embassy/Consulate Generals about the intention to visit restricted regions. It may take 2 weeks or longer for such permission to be granted (or denied). Visitors that enter these regions without permission are subject to arrest, detention, and will be prosecuted according to Indonesian law.
